Lighting Layout Request Form

The following information is required in order to complete a lighting layout for an indoor or outdoor lighting project. Please provide as much information as possible.

Customized report formatting for client preference. Adds lead time, please include details of how you would like the report formatted in the project description section.

Example: the lights near the windows to turn down when the sun is out. Lights at the workstations to only be on when an operator is working but turn off when nobody is present.
